Follow the steps below to begin filling out the initial forms so we can gather as much information to best serve you. 

 Step 1: Create an account by choosing a username and password to access our health portal:

Step 2: Once you create your account an email will be sent to the email used to create the account. Once you receive the confirmation email, click on the link in the email to verify your account.

Step 3: Once you verify your account you’ll be redirected back to the login page for our health portal. Use your new username and password to login.

Step 4: Finally, once you’re logged in you will see there are 3 forms to fill out and once finished you can sign and submit them and we will receive your forms. You also have the option to edit and fill out contact and demographic information, as well as any insurance information that might be useful.

Step 5: Then just call, email, or use our convenient online booking feature to book your initial appointment!!
